Medical Forms for Administering Medicine in School

If you are sending your child in to school and they require medicine to be administered during the school day (this must be at least a 4x a day requirement or before and after meals) you will need to complete a medical form when dropping the medicine off.  These forms are available from Breakfast Club and the school office.
